What does it take to successfully integrate behavioral health into primary care? This session explores two real-world models—one from a statewide collaborative in California and another from a rural Arizona clinic network—that demonstrate how data, technology, and team-based care can drive measurable improvements in access, quality, and outcomes. Learn how to build and scale behavioral health integration that works in both urban and rural settings.

Incorporating behavioral health into primary care is a proven strategy for improving patient outcomes, enhancing care team satisfaction, and reducing total cost of care. This session will show how health systems across California and Arizona are advancing behavioral health integration (BHI) through data-driven strategies, standardized workflows, and multidisciplinary care models. Speakers will discuss two complementary approaches: the CalHIVE BHI program, where eight California health systems are using NCQA HEDIS® measures to monitor impact and sustainability, and a rural Arizona network that achieved full integration across 17 clinics using shared EHRs, standardized screening tools, and coordinated care teams.

Attendees will learn how to define measurement strategies, implement integrated workflows, and sustain behavioral health integration across diverse settings. The session will offer practical tools, lessons learned, and scalable strategies to support whole-person care—whether in large health systems or resource-constrained rural environments.
